Vim のバッファにコピペで貼りつけた上で次のようにコマンドを叩くと…

こんなんなります。^M は <CTRL-V><ENTER> で入力しましょう。めんどくさければマクロ化や関数化しても良いですが、まぁその場で組み立てれば十分です。
